The Medical Records Department is one of the key and
indispensable departments in the hospital. It deals with all other key
units and without its services there can hardly be a smooth running of any hospital. More than 100,000 patients have opened files in SMCH so far , and many more are expected to do the same in the near future.

Functions:

The Medical Records Department operates 24 hours daily. It includes the following (6) sections:

1. Filing Section.
2. Chart assembling and analyzing Section
3. Lab Section.
4. Release of Information Section
5. Incomplete Section
6. Transcription section.

The transcription Section operates 12 hours daily. The transcriptionists transcribe physicians reports using the Dictaphone system ( the doctor makes the report over the phone ).

The filing Area operates 24 hours on a three shifts basis (morning, evening and night) and is responsible for pulling records needed for the following day and for both scheduled and walk-in patients in different clinics, iin-patients & ER.

Release of Information: This section serves all patients, companies, guarantor, and insurance companies who are requesting supporting documents or a chart copy.
